WeCare 365 Adult Foster Care program means we are committed to caring 365 days a year with care plans carefully tailored for member needs through a team approach for holistic, wrap-around services. We change lives one family at a time by recognizing their diversity and support them with professional staff who are multilingual and multicultural. WeCare 365 provides caregivers and member families with financial relief and the comfort that comes with comprehensive service. 

WeCare 365's vision over the next 3 years is to be serving a community of over 1500 members and their caregivers.  We will have a diversified member population that spans the age of eligibility beginning at age 16 and ending with the elderly.  We will continue to be a trusted community partner for our members, caregivers and also for the State, Insurance Companies and Medical Providers in order to improve quality of life while reducing overall medical expense.  We will be advocates for all entities that touch and influence the life and experience of those that we serve.
